Application for initial Nevada license (license fee pro-rated quarterly)
Quarterly Proration Schedule ($75 non-refundable application fee plus $93.75 for each calendar quarter remaining – Includes 2-year license at one practice location).

A completed renewal form, continuing education compliance information, and proper renewal fees must be submitted bearing a postmark on or before February 28th or 29th of each even-numbered year to avoid suspension of active and inactive licenses. A $500 penalty applies to untimely renewal
The period to renew a suspended license is 90 days, from the last day in February until midnight on May 29th. On May 30 at 12:01 a.m., the license and all related certificates will expire, and, to gain licensure again in the state, the optometrist must submit a new application for licensure and all information required by the application, including a passing score on the Nevada Law Exam.
Reinstatement Period if license suspended for non-renewal is 90 days. $750.00 2-year renewal fee, plus additional locations as applicable ($200.00 each), plus $500.00 late fee.
